Please plan to arrive about 15 minutes early to complete the appropriate forms prior to your appointment.
Financial Responsibility
If the patient is a minor, the parent or guardian who accompanies them to the visit will be responsible for the account.
Underage Patients
Any patient under 18 years of age must be accompanied by a parent or legal guardian. We are unable to see small children without the presence of a parent or legal guardian. For young adults, we are able to see you without a parent after your initial visit for your routine check-ups.
Co-Payments
(Your health insurance contract is between you and your insurance provider)1. Co-payments must be paid at the time of your visit and will be collected upon checkout. If you are unable to pay your co-payment, we will request that you reschedule your appointment to a later date.

Please have your insurance card ready prior to your appointment. Without your physical card, we will ask you to pay for your visit in full or reschedule your appointment.
Payment Options
1. Brookwood Dermatology accepts cash, checks, Visa, MasterCard, Discover and American Express.
2. Most cosmetic services are not covered by insurance companies. If your insurance does not cover the service you are receiving, payment must be made on the date of the service.
Missed Appointments
Call 205-824-4441 as soon as you know you will be unable to make your appointment. This allows another patient to schedule an appointment in your place.
Referrals
If your insurance requires a referral and you would like to see a dermatologist without one, you must pay for that day's visit or reschedule for a later date.
